The Data Rivr team developed a set of Clinical Dashboards to allow providers to view their patients' data to support care and chronic disease management.

Valuable Data

Using the HIE data as a base, the Data Rivr team setup services to receive ongoing data to feed to the Clinical Dashboards. The new/updated data was processed and run through algorithms to assess patient clinical status for the following registries:

Hypertension (Uncontrolled and Undiagnosed)
Diabetes (Undiagnosed and Prediabetes)
Osteoarthritis
Rheumatoid arthritis
Hospital and ED readmissions
Quality Measures
