Clinical Dashboards & HIE Data

Using the HIE data as a base, the Data Rivr team setup services to receive ongoing data to feed to the Clinical Dashboards. The new/updated data was processed and run through algorithms to assess patient clinical status for the following registries:

 

• Hypertension (Uncontrolled and Undiagnosed)

• Diabetes (Undiagnosed and Prediabetes)

• Osteoarthritis

• Rheumatoid arthritis

• Hospital and ED readmissions

• Quality Measures

As part of this project, the Data Rivr team utilized the following healthcare technologies:

 

• HL7 and FHIR standards

• CCDa document exchange

• Integrations with EHRs
